I am looking for some information regarding Major Adaptations processes in your Borough for the past 3-4 years, i.e. since 2013, specifically:

1. THE REFERRAL PROCESS – the process for applying for a DFG (Disabled Facilities Grant) after an OT has completed assessment that may result in a DFG
Referral received from an Occupational Therapist which details the recommended adaptation.
Council caseworker requests information from the disabled person to carry out the means test.
Result of means test advised to the person.
If the person is eligible for a grant, assistance is given to complete the grant application forms.
Quotes for the cost of the recommended adaptation are requested.
Once these are received, the grant is approved, notified by letter.
2. WAITING TIMES – the waiting times for the DFG to be processed
Receipt of referral to 1st contact – 2 – 4 weeks
Time scale of 1st contact to completion of works - +- 16 weeks
3. NUMBER OF ASSESSMENTS COMPLETED FOR THE DISABLED FACILITIES GRANT – the number of assessments which resulted in a recommendation for a DFG

Additionally, please supply information do you have, for instance annual reports regarding major adaptations since 2013. – No reports have been processed as our agent arranged the referral works in this period and we have no access to their records. They have since closed. Since April of this year we are processing the DFGs in house.
